چکیده

THE anatomic sites at which obstruction to the egress of blood from the left ventricle may occur are three in number, as follows (fig. 1): the aorta, the aortic valve, and the subaortic region of the left ventricle. Obstruction in the aorta may involve one of two general areas, namely, (1) the arch or just beyond, the lesions taking the forms of classical coarctation, tubular hypoplasia, or interruption of the arch, or, (2) it may involve the ascending aorta. As the clinical manifestations of obstructive lesions in the vicinity of the aortic arch are not usually confused with obstruction in the region of the aortic valve, the author has chosen not to consider these conditions further in this symposium. Rather, consideration to obstructive lesions of the aorta will be confined to those that involve the ascending aorta, lesions which, while varied in appearance, are usually congenital in nature and, collectively, may be termed supravalvular stenosis of the aorta. Obstruction at the aortic valve itself, termed either aortic valvular stenosis or simply aortic stenosis, may be represented by a congenital malformation or may result from acquired disease affecting either an initially normal valve or one that is the site of a congenital malformation; the latter most commonly takes the form of a congenital bicuspid valve. Obstruction occurring proximal to the aortic valve in the outflow tract of the left ventricle is commonly known as subaortic stenosis. This condition is represented by a wide

Severe volume depletion leading to mitral insufficiency.

We report a patient with profound hypovolemia who developed dynamic left ventricular outflow tract obstruction and severe mitral regurgitation. Both the outflow tract obstruction and mitral regurgitation resolved with volume replacement. Unlike previous reports of dynamic left ventricular outflow obstruction and mitral regurgitation, the degree of mitral regurgitation was severe. Possible mecha...

Transient dynamic left ventricular outflow tract obstruction in a patient with pheochromocytoma.

Symmetric left ventricular hypertrophy or asymmetric septal hypertrophy associated with pheochromocytoma simulating hypertrophic obstructive cardiomyopathy have been rarely reported. In this report, we present a case with pheochromocytoma that had dynamic left ventricular outflow tract obstruction without asymmetric septal hypertrophy. A surface echo revealed resolution of the systolic anterior...

Giant Left Atrial Myxoma Causing Mitral Valve Obstruction and Concomitant Coronary Artery Disease

We describe a 56-year old male patient who presented with congestive heart failure and had undergone echocardiography that showed a mobile, giant left atrial mass which caused obstruction in the left ventricular outflow tract. The patient underwent a coronary angiography before operation that showed severe stenosis in the left anterior descending and circumflex artery. The patient underwent res...

Dynamic left ventricular outflow tract obstruction: underestimated cause of hypotension and hemodynamic instability

Left ventricular outflow tract obstruction, which is typically associated with hypertrophic cardiomyopathy, is the third most frequent cause of unexplained hypotension. This underestimated problem may temporarily accompany various diseases (it is found in even <1% of patients with no tangible cardiac disease) and clinical situations (hypovolemia, general anesthesia). It is currently assumed tha...

Significance of left ventricular outflow tract cross-sectional area in hypertrophic cardiomyopathy: a two-dimensional echocardiographic assessment.

The morphologic determinants of subaortic obstruction in patients with hypertrophic cardiomyopathy are not completely understood. To define the relation between left ventricular outflow tract orifice size and presence or absence of subaortic obstruction, we studied 65 patients with hypertrophic cardiomyopathy and 16 normal controls by quantitative two-dimensional echocardiography. Left ventricu...

A 90-year-old patient presenting with postoperative hypotension and a new murmur: a case report

INTRODUCTION Hospitalists are frequently consulted on postoperative patients with hypotension. Postoperative hypotension is common and can be due to variety of causes. Systolic anterior motion of the mitral valve leading to left ventricular outflow tract obstruction is a rare cause of postoperative hypotension and can occur without prior structural heart disease. A high index of suspicion can l...
